It’s always been from restaurants as far as I remember. I’ve only ever heard it in restaurant contexts; i.e. “we’re 86 French fries” or something like that.

86 is shorthand for “removed from the menu” or similar.

I always heard it as “8 miles out, 6 feet under” I.e murdered and body dumped ages away.
